We make referral recommendations:

Physical Therapy Services
Specializes in improving functional limitations, which involves establishing a home exercise program designed to improve mobility, strength, range of motion, and pain management. Physical therapists may also educate caregivers with the goal of expediting healing and preventing re-injury.

Home Delivered Meals - Meals on Wheels Program
Provides appropriate and nutritionally balanced hot, cold, or frozen meals served in the home with flexible options including: low fat/low sugar, diabetic, low sodium, pureed, and ethnic preferences, such as Kosher, American, and Spanish.

Medical Equipment
Includes the provision of assistive devices, such as special recliners, hospital beds, walkers, wheelchairs, canes, and adaptive telephone devices.

Consumable Medical Supply Services
Includes the provision and delivery of usable supplies such as adult disposable diapers, personal wipes, lotions, bandages, nutritional supplements, and other consumable supplies.

JC's Home Health Care Service and programs available through our funding sources
are designed to assist individuals who are functionally impaired and cannot perform
the normal tasks of daily living. Most government and privately funded programs have specific eligibility and availability requirements. Individuals who are able to pay for services are served through our Private Pay program. This program also serves as
a short-term alternative for individuals who are eligible for assistance, but need
immediate support.

Community Care for the Elderly

State program. Serves clients 60 years of age or older who are functionally impaired. A client is deemed functionally impaired if he/she is unable to perform the normal tasks of daily living. CCE clients pay a co-payment based on a sliding scale.This includes ESI (Emergency Services Intervention) / APS (Adult Protective Services) clients. Program services include: Adult Day Care, Case Aide, Chores, Case Management, Companionship, Counseling, Emergency Alert Response, Enhanced Chore, Escort, Home Delivered Meals, Housing Improvement, Homemaker, Nutritional Risk Reduction, Personal Care, Pest Control, Respite, Specialized Consumable Supplies, and Medical Equipment.

 

Medicaid Waiver (Home and Community Based Services Waiver)

Combined State and Federal program. Serves clients 60 years of age or older who are homebound and at risk of institutional placement. Clients must meet Medicaid income level requirements and asset ICP limits. The gross Monthly Income Limit for Medicaid applicants is $1,869. The Asset Limit for an individual is $2,000 and for a couple is $3,000. Program services include: Adult Day Care, Adult Day Health Care, Case Aide, Chores, Case Management, Companionship, Counseling, Medical Equipment, Emergency Alert Response, Environmental Modification, Escort, Home Delivered Meals, Homemaker, Nutritional Risk Reduction, Occupational Therapy, Personal Care, Pest Control, Physical Therapy, Respite, Specialized Consumable Supplies, Skilled Nursing, and Consumer Directed Care Plus (CDC+).

Assisted Living Waiver

Combined State and Federal Medicaid Waiver program. Serves clients 60 years of age or older with the ability to stand and walk requiring minimal assistance who live in an Assisted Living Facility. Income requirements are the same as for Medicaid Waiver. Balance of funding authority goes to the facilities to pay for home care services. Program services include: Assisted Living Facilities, Case Management, Specialized Consumable Supplies, and Medical Equipment.

Home Care for the Elderly

State program. Serves clients 60 years of age or older and encourages the provision of care in family-type living arrangements in private homes on a not-for-profit basis as an alternative to a nursing home or other institutional care. Must meet same income requirements as Medicaid Waiver and require 24 hour care and have a live-in caregiver. Program services include: Basic Subsidy, Case Aide, Case Management, Medical Equipment, Environmental Modification, Respite, Specialized Consumable Supplies, and Special Subsidy.

Community Care for Disabled Adults

State Program. Serves clients 18-59 years of age who are functionally impaired. Program services include: Chores, Case Management, Home Delivered Meals, Homemaker, Personal Care, Respite, Specialized Consumable Supplies, and Medical Equipment.

Community Care for Disabled Adults Medicaid Waiver

Combined State and Federal Medicaid Waiver program. Serves clients 18-59 years of age who are functionally impaired. Must meet same income requirements as Medicaid Waiver. Program services include: Case Aide, Chores, Case Management, Counseling, Medical Equipment, Escort, Home Delivered Meals, Homemaker, Nutritional Risk Reduction, Occupational Therapy, Personal Care, Pest Control, Physical Therapy, Respite, and Specialized Consumable Supplies.

Nursing Home Diversion Long Term Care Program

Combined State and Federal Program. Serves patients 65 years of age or older; Medicaid recipient or eligible; enrolled in Medicare Part A and B. Program for those individuals considered at risk for nursing home placement, in need of daily supervision/help with activities such as bathing, dressing, eating, toileting, transferring or walking/mobility, or who have a chronic health condition which requires daily nursing services. The Solutions program provides additional community care and medical services not available under standard Medicare and Medicaid HMO programs. Program services include: 24 Hour On-Call, Adult Day Health Care, Assisted Living Facilities, Chores, Case Management, Companionship, Counseling, Medical Equipment, Emergency Alert Response, Enhanced Chores, Environmental Modification, Escort, Home Delivered Meals, Home Health Aide, Housing Improvement, Homemaker, Nutritional Risk Reduction, Occupational Therapy, Personal Care, Pest Control, Physical Therapy, Respite, Prescriptions, Specialized Consumable Supplies, Skilled Nursing, TeleHealth, Dental, Vision, and Hearing.

Private Insurances

We accept private long term care insurances. Clients may receive skilled home health care as prescribed by their physicians and under the supervision of a registered nurse. Program services depend on type of insurance. Available services may include: Adult Day Care, Adult Day Health Care, Case Aide, Chores, Case Management, Companionship, Counseling, Medical Equipment, Emergency Alert Response, Enhanced Chores, Escort, Home Delivered Meals, Home Health Aide, Housing Improvement, Homemaker, Nutritional Risk Reduction, Personal Care, Pest Control, Respite, Specialized Consumable Supplies, Skilled Nursing.

Medicaid/Medicare

State and Federal programs. Clients may receive nursing, therapy, home health aide, and medical social work services as prescribed by their physicians and under the supervision of a Registered Nurse.

Older Americans Act III-B

Federal program. Older Americans Act Supportive and Access Services. Serves clients 60 years of age or older and homebound. Program services include: Case Aide, Chores, Case Management, Companionship, Enhanced Chores, Environmental Modification, Escort, Housing Improvement, Homemaker, and Personal Care.

Older Americans Act III-E

Federal Program. Older Americans Act Family Caregiver Support Program. Serves caregivers providing care to older adults age 60 or older. Program services include: Respite, Special Consumable Supplies, and Medical Equipment.

FEMA Emergency Food and Shelter Program

Federal program. provide short term services such as home delivered meals, emergency nutritional supplements, and hurricane shelf stable meals. Serves elderly ages 60 years of age and older and disabled adults ages 18-59.

Community Based Home Care Services for Disabled Adults

Local Program. Serves disabled adults ages who have significant debilitating injuries and health conditions, who have physical sensory motor and/or developmental disabilities, and who are unable to fend for themselves or meet basic needs of daily existence. Provides direct services to at risk clients whose care plans have been reduced due to funding cutbacks, allows for specific services not covered under current funding, and serves high risk clients with no source of funding. Program services include: Adult Day Care, Case Aide, Chores, Case Management, Companionship, Counseling, Medical Equipment, Emergency Alert Response, Enhanced Chores, Environmental Modification, Escort, Home Delivered Meals, Homemaker, Nutritional Risk Reduction, Personal Care, Pest Control, Respite, and Specialized Consumable Supplies.

 

Community Based Home Care Services

Local Program for frail elders age 60 years of age or older who are at highest risk for nursing home placement residing at home or in a community setting. Program services include: Adult Day Care, Adult Day Health Care, Case Aide, Chores, Case Management, Companionship, Counseling, Medical Equipment, Emergency Alert Response, Enhanced Chores, Environmental Modification, Escort, Home Delivered Meals, Homemaker, Nutritional Risk Reduction, Personal Care, Pest Control, Respite, and Specialized Consumable Supplies.

Caregiver Counseling, Education, and Support

Local Program to provide caregiver support, counseling, and education. It provides concurrent respite at home or in an Adult Day Care setting. The caregiver support programs are aimed at providing techniques for caregivers to reduce burden and relieve stress and targets caregivers based on need with priority given to those on the waiting list for services. Provides counseling, education and emotional support to caregivers. Program services include: Adult Day Care, Case Management, Counseling, and Respite.

In-home Services

Community Based Organization local program.This program provides care management and a variety of in-home services including homemaker and personal care to frail elderly (60+) in order to maintain community tenure and support family care giving. Program services include: Case Management, Home Delivered Meals, Homemaker, Personal Care, Specialized Consumable Supplies, and Medical Equipment.

 

In-home Services for Developmentally Disabled Adults

Community Based Organization local program. This program serves developmentally disabled adults ages 18–59 throughout all St Louis MO. Program services include: Case Management, Homemaker, Personal Care, Respite, Specialized Consumable Supplies, and Medical Equipment.
